About this workshop
This workshop delved into the work carried out in the Staffordshire and Stoke-on-Trent system to redesign and transform their CHC service. This was driven by an escalation of costs and a desire to improve patient care.
The workshop roughly followed the below agenda:
- finance scene setting
- patient story
- outline of the project and its aims
- provider collaborative - the undertaken on transferring and redesigning commissioning and delivery
- overview of individual programme workstreams:
i. high-cost packages
ii. discharge to assess
iii. clinical and finance benchmarking
iv. market engagement
v. 1:1 care
vi. choice policy
vii. local authority working relationships
viii. personal health budgets - open forum for questions.
