Addressing healthcare inequalities through NHS finance action: End of Life Supportive Care

There is huge inequity of access to Palliative Care, mostly on the basis of diagnosis, with most Specialist Palliative Care prioritised to cancer patients. Senior clinicians in Cardiology and Palliative Care recognised the significant gap in quality of end of-life patient care for heart failure patients, and initiated a pilot. The Heart Failure Supportive Care Service (HFSCS) operates from the principles of co-production to establish a personalised and cost-effective model of care, which better suits the needs and wishes of each patient; recognising the patient’s preferred place of care and death.  Working closely with the clinical team, the FBP team assisted in securing permanent funding for the HFSCS. This was achieved by supporting them to navigate the organisation’s governance processes, by writing the business case and advocating for the development. More recently, the finance team has undertaken a detailed evaluation of the impact of the initiative, which provided a robust and independent quantification of its value-based benefits.  The active involvement of the FBPs and the evaluation of the HFSCS has been crucial, not only to ensure its continuity, but also to facilitate scalability to allow wide-scale health inequalities in palliative care service provision to be addressed. The team has secured the support of Executives to expand the model within Heart Failure and to three other clinical areas (nephrology, respiratory and hepatology). In addition to expansion within CAVUHB, the impact of HFSCS is also being shared across Wales. This has resulted in the HFSCS model being piloted in 4 other Health-Boards with excellent outcomes.

Lunch 

The lunch will be held the Classroom restaurant, menu choices will be available during booking registration. 
All attendees will also be provided with one drinks voucher for a glass of wine, a beer or a soft drink.
